​​Please bring your most current glasses, your photo ID, and medical insurance card to your appointment.

If you wear contact lenses, please wear them into your appointment, and bring your contact lens information with you.

Insurance Plans

It is important to know that the information provided by your insurance company DOES NOT guarantee payment, therefore you, the patient are ultimately responsible for all services rendered. Please call ahead to with any questions regarding your specific plan.

medical plans

We are providers for medically based eyecare for most of the plans listed below. In addition, several of the medical insurance carriers may also cover “routine” eyecare (like Aetna and Anthem).

  • Medicare

  • Aetna

  • Anthem

  • Cigna

  • Humana

  • UMR

  • United Healthcare

VISION plans

  • We are an out-of-network provider for EyeMed and Vision Service Plan (VSP). Even as an out-of-network provider, you will still be able to use your out-of-network vision benefits at Highland Eye Boutique. 

  • If you are planning to use your medical insurance benefits, please be familiar with the coverage of your policy. One of the easiest ways to learn about your coverage is to speak with a representative from your insurance company. Below is a list of questions you may want to ask when calling:

    • Do I have “Routine” eye care coverage through medical insurance?

    • How often am I covered for an eye examination?

    • What are my co-pay and deductible amounts?

    • Is Highland Eye Boutique listed on my insurance as a “Participating Provider”?

    • Do I have out of network benefits to use towards glasses and/or contact lenses?
